Nurse Practitioner, Addiction Medicine Program Full Time Position (3-year contract with the possibility of extension)

Duties & Responsibilities:

• Develop and lead educational and training workshops related to addiction medicine for employees of Anishnawbe Health Toronto as well as other organizations

• Consult internally at AHT and externally to triage potential addiction medicine clients

• Facilitate and oversee the intake process for the Rapid Access Addiction Medicine Clinic and interface with all primary care services in and outside of AHT; assist with the evaluation process for the Clinic

• Perform clinical duties within scope of practice including assessment, diagnosis, counselling, education, advocacy, health promotion, treatment and prevention, internal and external referrals and follow-up

• Determine appropriate healing path in collaboration with the client and other health care providers on the multidisciplinary team

• Proactively identify and address community health issues

• Develop and maintain good working relationships with external organizations that contribute to the health and wellbeing of clients

• Ensure professional competency through ongoing education, training and literature review Qualifications & Experience:

• Masters Degree in Nursing

• Current registration in the Extended Class (RNEC) with the College of Nurses of Ontario

• Successful completion of a Controlled Substances Prescribing course

• A comprehensive understanding of addictions and Indigenous health issues

• Two years of clinical experience with mental health and addictions preferred

• Well-developed skills in self-direction, interpersonal communications, collaboration, critical thinking, problem-solving, conflict resolution and scholarly inquiry

• An understanding of the social determinants of health and other issues relevant to Aboriginal communities in Toronto; respect for Traditional healing
